Root pin prosthesis

The root canal post denture is such a prosthesis which replaces the tooth’s crown or a part of it with the help of a pin guided into the root canal. The extended crown defects can be reconstructed with this method. Root pin can be inserted only in root canal treated teeth. The pin is responsible for anchoring the crown abutment. If your tooth’s crown part is not suitable for preparing crown; and the bone around the root is sound and there is no inflammation, then we the pin insertion is recommended into the root.

Usually with pin denture we don’t restore the original shape of the crown, but we make a so called artificial abutment which looks like a polished tooth and we separately prepare the covering crown into this, and then fix it with cement.

When we recommend preparing root pin?

  • tooth fracture – only the crown part has damaged, but the root remained sound
  • as a result of tooth decay the tooth is largely damaged, destroyed the crown part of tooth
  • if we prepare the tooth for crown, but the abutment is smaller than 3 mm
  • if we want to correct the tooth alignment

Conditions of the preparation:

  • the root is minimum 8-10 mm long
  • the length of 2/3 of root can be used for anchoring the pin
  • perfect root filling which easily can be removed if necessary

When can’t be applied:

  • if the root is curve
  • if the root is too short
  • if a part of the tooth is broken under the gum
  • in the case of abnormal process around the apex
  • not perfect, the root filling doesn’t reach the apex
  • if the tooth can be moved due to periodontal disease
  • childhood – if the root development isn’t completed, because in this case preparing a perfect root filling is not possible
  • elderly – if the root canal is too narrow, the dilatation is difficult

Types of root pin:

  • Prefabricated factory pin – in several sizes available. With the help of this we can make very easily and quickly root pin dentures. Its disadvantage is that it does not fit exactly into the root canal; therefore its immovableness is not perfect. It can be made from metal or plastic reinforced with carbon fiber which is white colored, so it is more aesthetic. The most modern is the fiberglass pin.

The process of preparation:

Root pin can be placed only in root canal treated tooth and into tooth with root canal filling. The X-ray gives information about the tooth’s status. Therefore, if there is a possibility to choose this type of restoration, then the X-ray is always necessary from the given tooth. Based on the record we get information about the root filling, about the length of root canal and its “shape”, is it curve or not, there is some root apex lesion or not, which may contraindicate the procedure.

If necessary, we make root canal treatment, and perfectly fitting filling will perform with an appropriate length; the root apex lesions will be also treated. In the case of new root filling the pin can be inserted at least 24 hours later.

Into the filled root we prepare a hole for the pin with the help of special drills. We take an impression from this hole, and then the impression will be sent to the dental laboratory where it is poured out with plaster and they make the sample. On this plaster sample will be finished the work, the root pin denture. The root pin prepared by the dental technician fits perfectly in the root canal; therefore it provides adequate stability for the superstructure (crown). In the following session we check the fitting of the pin, and if it is satisfactory, we glue it into the root canal with the help of adhesive cement. This is followed by the preparation of the covering superstructure. This is necessary because the root pin denture’s mouth part patterns a polished tooth. This must be fitted with a covering crown.

Again we take an impression – together with the bonded root pin denture. The impression is poured out with plaster in the dental laboratory. The crown is prepared to the given sample. Next time we rehearse this crown. If it is properly fit, and you are also satisfied with it we fix it to the root pin prosthesis wit adhesive cement.
